Patient Sign Up - FREE
*You are a patient who wishes to create your own electronic health record which your healthcare providers may view and add information to.
*You have been given a “Confirmation Code” by your healthcare provider, and you wish to view, edit or add information to your existing WebDMEMR medical record.
Health Care Provider Sign Up - First 30 days FREE
*You are a healthcare provider who wishes to create a WebDMEMR account for your practice.
*You are a site administrator who wishes to create a WebDMEMR account for your hospital or multi-physician practice.
*As a Head of Household you may use the health care provider sign up and pay only the user fee to create and manage your family's electronic health records.
*You need an electronic medical record for your telemedicine network.
*You need an electronic medical record for clinical trials.
